ACCM Fellowship Match

The majority of ACCM Fellowship Programs participate in match administered through SF Match. Each program varies in the number of available positions and the interview process. For program-specific details, please refer to the individual program’s website. Information on participating programs, match enrollment instructions, and the match timeline can be found on the SF Match website.

Common Application Form

All ACCM fellowship programs participating in SF Match use the SF Match Central Application Service (CAS). The application is available on the SF Match website.

The common application form includes a personal information form, required supplemental form, requisite letters of recommendation, and a personal statement. Applicants should check with individual programs for any additional supplemental material requirements.

Information for Applicants Applying to the Dual Anesthesiology Critical Care Medicine and Adult Cardiothoracic Anesthesia Fellowship Program

The CCM and ACTA fellowship program directors have collaborated to develop materials to assist applicants interested in dual training in critical care medicine and cardiac anesthesiology. Please review this informational document for guidance on the application process. Additionally, consider submitting this supplemental questionnaire to programs as recommended.

Core Program Director Standard Letter of Recommendation

In place of a letter of recommendation from the Chair or Program Director, we encourage Core Anesthesia Program Leadership to use the Letter of Evaluation developed by the Association of Anesthesiology Subspecialty Program Directors. This form provides programs with an objective assessment of a trainee’s performance during residency. It also includes a free-text section for a summary evaluation and written comments supporting a candidate’s application. SOCCA Match Exceptions

SOCCA recognizes an Exception Agreement process, which allows for early agreements between programs and applicants before rank list submission. However, this process must strictly adhere to the timeline established by the SOCCA Program Directors Advisory Council (PDAC).

Six specific circumstances exist in which a program and an applicant may choose to use an Exception Agreement. These exceptions are designed to accommodate unique situations while maintaining fairness in the application process. Importantly, all programs and applicants must still register with SF Match and submit their rank lists through the official match system, even if an Exception Agreement is used.

The SOCCA Fellowship Program Directors Advisory Council has formally approved the six exception criteria. Additional details about match exceptions are outlined below, and programs and applicants should review the Critical Care Anesthesiology Fellowship Match Exceptions Policies for 2025 for further guidance.

Applicants may participate in a Match Exception Agreement with a single program if they meet any of the following criteria:

  1. Active military service at the time of application.
  2. Making a commitment to come to the institution of the ACCM fellowship for more than one year (for example, second fellowship year for EM candidates, Anesthesiology Cardiothoracic Anesthesia, or other approved second year).
  3. Enrolled in an anesthesiology residency outside of the USA at the time of the application.
  4. Residing outside of the USA at the time of the application or who are not eligible for ABA certification due to non-US training.
  5. Spouse or partner applying for a GME-approved post-graduate training program in the same region.
  6. Internal candidates (i.e., applicants who are currently in the anesthesiology or emergency medicine residency programs at the same institution as the ACCM fellowship).

To complete the SOCCA Exceptions to the Match Process:
  1. The Program Director must complete and submit the Program Exception Agreement Form.
  2. After submission, the Applicant will receive a confirmation email with a link to the Applicant Exception Agreement Form.
  3. Once the Applicant completes their portion of the agreement, the Program Director and the Applicant will receive a final confirmation email indicating the process is complete.

The agreement is only finalized once both parties have submitted their completed agreement forms and each party has received a confirmation email.

Approved exception agreements will be posted on the website once the exception process opens.

Please note: Eligible applicants and programs who wish to use the exception agreement pathway are still required to participate in both the exception agreement form through this SOCCA webpage and the SF Match ranking process. Any match irregularities will be referred to the SOCCA Program Directors Advisory Council leadership.

Match Exception Violation Form

The Program Directors Advisory Council and SOCCA Board of Directors are committed to supporting a fair, equitable and successful match process for the candidates applying for the Critical Care Anesthesiology Fellowship programs. We have developed and supported match exception agreements to allow for the coordination of unique situations that can not be readily accommodated throughout the match.
